As of November 30, 2025, we will no longer be participating with Humana insurance plans. We have experienced numerous incidents of non-payment of claims for services that we have provided for our patients under this plan. This has caused significant financial hardship for our practice, staffing concerns, and an inability to provide quality care to our patients.

  • What happens now?

    Nothing will change for our existing patients right now. Existing patients should have ample time to complete their treatment and reach their goals, since our Humana contract will not lapse until November 30, 2025. However, you will need to either change insurance carriers or pay for services if appointments are needed after the termination date.

  • What does “Out of Network” mean?

    Providers who are “out-of-network” are those who do not participate in that health plan’s network. After our Humana contract ends on November 30, 2025, ENT Associates of Manatee will not be in-network with Humana. This means that ENT Associates will no longer be able to submit claims on behalf of our Humana patients.

  • What does “Self Pay” mean?

    Existing patient who wish to continue care after November 30,2025, will be converted to “Self Pay” payment system. This means you will pay a flat fee at the beginning of your appointment and upon check-out, any procedures that were completed during your appointment. Our rate for new patients is $350.00 at check-in and $200.00 for existing patients and follow-up appointments. If you have “Out of Network” benefits, you can personally seek reimbursement from Humana after your visit.
